What is Semaglutide?
Semaglutide (SMGB12) is a synthetic form of the natural hormone Glucagon-like Peptide-1. It helps you feel
full by acting on appetite centers in the brain and gut. Participants in the STEP1 study lost an average of
14.9% of their body weight and saw improvements in blood sugar, heart health, and overall quality of life.
What are the side effects of Semaglutide (SMGB12)?
Side effects vary. Some people experience no symptoms. Others may have nausea, vomiting, diarrhea,
constipation, heartburn, fatigue, or headaches.
What are the potential risks associated with Semaglutide (SMGB12)?
Possible risks include gallstones, thyroid tumors (in rodents), kidney injury from dehydration, and allergic
reactions.
Can Semaglutide (SMGB12) cause low blood sugar (hypoglycemia)?
Yes, especially if you’re also on insulin or sulfonylureas. Medication adjustments may be required.
Who should not use Semaglutide (SMGB12)?
Do not use if you or a family member has a history of Medullary Thyroid Carcinoma or MEN II. Also avoid if
pregnant, planning pregnancy, or breastfeeding. Stop immediately if you become pregnant and contact
Scale-Smart Telehealth.
How do I administer Semaglutide (SMGB12)?
Keep Semaglutide refrigerated. It is a once-weekly injection using insulin syringes. Doses typically start at
0.25mg or 0.75mg weekly and increase by 0.25mg every four weeks. Your dosing card will specify your exact
schedule
Can I increase my dose faster than the recommended schedule?
Yes, but faster increases raise the risk of side effects and shorten how long your vial lasts. Follow the
schedule to reduce nausea, constipation, and other GI symptoms.
Do I need to increase my dose to the maximum recommended amount?
Not necessarily. You may stay at a maintenance dose if you’re losing weight with minimal side effects. Only
increase if weight loss plateaus.
Is there a time limit on how long I can take Semaglutide?
There is no specific time limit. You can continue beyond 12 weeks if needed. Note: the cost per vial stays the
same, but higher doses may make each vial last a shorter time.
What is Tirzepatide + Levocarnitine?
It’s a treatment plan that combines Tirzepatide, a weekly injection that helps with weight loss and blood sugar
control, with Levocarnitine, a nutrient that helps your body turn fat into energy. Together, they may boost
results for weight management.
How is it given
- Tirzepatide: A simple once‑a‑week injection under the skin.
- Levocarnitine: Can be taken as a pill, liquid, or sometimes an injection.
Your provider will recommend the best option for you.
What are the side effects?
- Tirzepatide: nausea, vomiting, diarrhea, constipation, or stomach upset. Rarely, it may cause
pancreatitis or gallbladder issues.
- Levocarnitine: upset stomach or a fishy body odor in some people.
Who should not use Tirzepatide?
Avoid tirzepatide if you:
- Have a family history of thyroid cancer (MTC) or MEN2
- Are pregnant or planning pregnancy
- Have had a serious allergic reaction to it before
Are there risks with the combination?
Most people do well, but some may have more stomach upset. Tirzepatide can rarely cause pancreatitis, and
levocarnitine may need caution in people with kidney problems.
Can I take Tirzepatide long-term?
Yes. Tirzepatide is meant for long‑term use, but your provider will check in regularly to make sure it’s safe
and effective for you.
Are lab tests required?
Labs are sometimes recommended but optional. If needed, we’ll email an invoice and place your order once
paid. You can walk into any nearby lab.
Does insurance cover these medications?
No. Scale-Smart Telehealth does not bill insurance, Medicare, or Medicaid.
How does the program work?
After your video consultation, medications and supplies are shipped directly to your home. The ultra-fine
insulin syringes minimize discomfort and feel like a quick pinch.
What is the cost of the Scale-Smart Weight Loss Program?
The program costs $399 per month (4 weeks) and includes consultation, chart review, assessment, medication
shipping, supplies, and follow-ups. You can pay monthly or use the Super Saver option: $1100 for the entire
12-week program, saving nearly $100.
How long will it take to receive my medication?
Once your signed consent forms and payment are received, your medication typically arrives in 3-5 business
days.
How often do I need follow-up appointments?
The first follow-up is 3-4 weeks after starting injections, and the second is at 10-12 weeks. All are virtual.
How do I schedule a consultation appointment?
Book your appointment through the
Scale-Smart Telehealth website or by calling us. Intake forms will be emailed after scheduling. Forms must be
completed and submitted at least 24 hours before your appointment. If not, your slot may be given to someone on
the waitlist. Signing consent forms does not commit you to treatment
